Though some surgeons offer facelifts under local anesthesia or “twilight” sedation in their offices, this practice is becoming less common due to advancements in anesthesia science. My preference is rooted in safety, predictability, efficiency, and sterility.
A surgeon's primary focus should be on anatomy and achieving the best possible results, not on managing discomfort as local anesthesia or light sedation wears off. Movement during surgery can create complications. While twilight sedation may be suitable for shorter procedures like colonoscopies, I prioritize comfort and full airway protection for facial cosmetic surgeries, minimizing any interference from the anesthesiologist or patient during the procedure.
As far as the surgery itself, a mini facelift typically lasts about 3 to 4 hours, including placing the final dressing. Drains are usually unnecessary, and incisions are carefully placed around the ears.
